Loved One Support
We support families in navigating the daily challenges of caring for loved ones with disabilities or chronic illnesses, offering dedicated care management, oversight, and compassionate, ongoing guidance for improved quality of life.
Who Qualifies
Be 16 years of age or older
Be eligible for MassHealth insurance coverage
Have a medical or mental condition that requires help with at least one daily activity, such as walking, eating, dressing, toileting, or transferring (from bed to chair)
Be clinically approved by their primary care physician and MassHealth
Our team will gladly guide you through each step of the eligibility process.
