Members or Clients Must:
To qualify for our program, members should meet the following requirements.
Be 16 years of age or older
Be eligible for MassHealth insurance coverage
Have a medical or mental condition that requires help with at least one daily activity, such as walking, eating, dressing, toileting, or transferring (from bed to chair)
Be clinically approved by their primary care physician and MassHealth
Our team will gladly guide you through each step of the eligibility process.
